Straightforward, g htforward, safe, and efficient interlocking g screw insertion during g intramedullary nailing g using g a Steinmann pin and hammer: a comparative study

- Abstract
- Objectives: Accurately targeting g etin g distal nail holes and placing g distal interlocking g screws pose challenges g es during g intramedullary nailing. g . This study proposes a straightforward g htforward technique q ue for distal locking g screw insertion using g a Steinmann pin, eliminating g the need to reposition the pin or drill bit. Methods: We utilized 18 Sawbones femur models and intramedullary femur nails. A first-year resident created two distal locking g holes on each model, employing g both the conventional freehand technique q ue and a novel method involving g a Steinmann pin and hammer under image g e intensification. These techniques q ues were evaluated based on three parameters: ( 1 ) the time required q uired to create distal locking g holes, measured from the moment the pin was positioned at the center of the hole until the far cortex was drilled through g h the interlocking g hole;(2) ; ( 2 ) the radiation dose ( in mrem/h), ) , as estimated with a personal g amma radiation dosimeter; ; and ( 3 ) the number of failures, defined as the creation of more than one hole in the near and far cortex. Results: The new technique q ue was associated with a lower radiation dose ( P=0.0268 ) and fewer failures ( P=0.0367 ) than the conventional approach. Additionally, the time required q uired to establish distal holes was shorter using g the new technique q ue compared to the conventional method ( P=0.0217 ) . Conclusion:The The creation of distal interlocking g holes with a Steinmann pin and hammer is accurate, efficient, and cost-effective.
