- Abstract
- 식도와 십이지장의 결핵은 위장관 결핵 중에서 드문 형태이다. 식도결핵의 증상은 삼킴곤란이 대부분이고, 십이지장 결핵은 특이적인 증상이 없는 경우가 많다. 식도결핵의 합병증으로 기관 식도샛길, 종격동 식도샛길, 대동맥 식도샛길 등이 있으며, 십이지장 결핵의 합병증으로 위날문막힘증, 주위 조직과 샛길 형성, 천공 및 상부 위장관 출혈 등이 있으며 다량의 상부 위장관 출혈은 드물다. 저자들은 토혈과 흑색변을 주소로 내원한 55세 남자 환자에서 시행한 상부 소화관내시경과 조직 생검, 방사선 검사에서 폐결핵에 의해 속발된 식도와 십이지장 결핵과 이에 동반된 십이지장 샛길을 진단하고 항결핵제 치료 후 호전되었던 증례를 경험하였기에 문헌고찰과 함께 보고한다.
