"Borderline Personality Traits and the Association With Sleep Quality and Depression in Medical Students"

Objectives: This study aimed to investigate the association between borderline personality traits and various psychological factors, particularly insomnia, chronotype, and defense styles, among medical students, a high-stress, non-clinical population. We specifically examined the role of depression in the association between BP traits and insomnia. Methods: A retrospective cross-sectional study was conducted with 139 medical students (97 male, 69.8%) at Gyeongsang National University. Participants completed self-report measures, including the Personality Assessment Inventory-Borderline Scale (PAI-BOR), Insomnia Severity Index (ISI), Composite Scale of Morningness (CSM), Korean Defense Style Questionnaire (K-DSQ), and Beck Depression Inventory (BDI-II). Significant predictors of PAI-BOR were identified using a series of multiple linear regression analyses. Results: Insomnia was a significant predictor of BP traits (PAI-BOR) in the initial multiple regression analysis (β=0.313, p< 0.001). However, in the multiple linear regression model controlling for depression, anxiety, age, gender, and other variables (e.g., resilience, chronotype), the significance of insomnia was lost (p=0.80), and depression remained the only strong predictor (β=0.417, p=0.001). Meanwhile, chronotype was not associated with PAI-BOR scores (p=0.228). Regarding defense styles, the self-inhibiting defense style significantly predicted increased PAI-BOR scores (β=0.38, p=0.03). Among individual defense mechanisms, Withdrawal (β=0.31, p=0.002), Projection (β=0.23, p=0.05), and Undoing (β=0.20, p=0.04) were found to be significant predictors for BP trait. Conclusion: The relationship between BP traits and insomnia appears to be largely explained by comorbid depressive symptoms. Furthermore, BP traits were significantly associated with a self-suppressive defense style. These findings suggest that assessing and managing underlying depression is crucial when addressing sleep problems in medical students with borderline personality traits.
