Factors Influencing Preferences for Decision-making Authority in Life-sustaining Treatment: A Comparison Between Middle-aged and Older Adults

This study aims to identify the preferred decision-maker for life-sustaining treatment among middle-aged and older adults, and to examine the factors influencing their preference. Data from 1,484 respondents in the Perceptions of Well-Dying survey were analyzed using logistic regression. The most preferred decision-maker for life-sustaining treatment was the individual themselves. Factors influencing preference for the decision-maker included gender, educational attainment, and views on life-sustaining treatment, which were common across middle-aged and elderly groups. In the middle-aged group, awareness of hospice and palliative care was found to be a predictor of selecting oneself as the decision-maker, whereas in the older adult group, the number of children was associated with the preference for self-decision-making. Based on these results, alternatives were proposed to enhance self-determination in end-of-life care.
