Anticoagulant Management of Persistent Vitelline Venous Aneurysm and Thrombosis in a Preterm Infant: A Case Report

A persistent vitelline vein is a rare vascular anomaly that may be complicated by thrombosis. Extension of thrombus into portal veins can result in portal hypertension, for which prompt surgical intervention has generally been considered the preferred treatment. We report a preterm infant with a persistent vitelline vein presenting with aneurysmal dilatation and thrombus extending into the main portal vein. Prenatal ultrasonography (USG) revealed a nonspecific intra-abdominal cystic lesion of unclear origin; however, postnatal USG and computed tomography subsequently confirmed a vitelline venous aneurysm with thrombosis. The patient was successfully managed with anticoagulation therapy, resulting in complete thrombus resolution without surgical intervention.
