Comparison of Perioperative Hemoglobin Changes by Anesthesia Type and Measurement Method in Hip Surgery

초록

This study compared perioperative hemoglobin(Hb) changes and measurement methods—central laboratory(Lab Hb), arterial blood gas analysis(ABGA Hb), and noninvasive pulse co-oximetry (SpHb)— by anesthesia type(general, spinal) in 240 patients undergoing total hip replacement. A retrospective analysis of electronic medical records confirmed significant postoperative Hb reductions across all methods. ABGA Hb showed strong concordance with Lab Hb (mean difference <1g/dL; agreement >96%), supporting its intraoperative reliability. Although SpHb underestimated Hb by ~1.35g/dL, it maintained over 95% agreement in tracking trends, with no significant difference by anesthesia type. These findings suggest ABGA Hb as a rapid alternative for transfusion decisions, while SpHb enables real-time trend assessment. Integrating these tools into anesthetic nursing protocols may provide objective evidence to support collaborative transfusion decision-making and enhance perioperative patient safety.
