| dc.description.abstract | 베르니케뇌병증은 비타민B1 결핍으로 발생하는 뇌질환으로 의식 저하, 조화운동불능 및 안구운동장애를 특징으로 한다. 만성 알코올 중독을 비롯해 비타민B1 섭 취가 부족하거나 대사에 이상이 있는 경우 발생하며, 진단 즉시 비타민B1을 투여하는 것이 신경계 증상을 개선하고 후유증을 줄이는 데 도움이 된다. 반면 시신경척수염범주질환은 중추신경계 염증탈수초질환으로 척수염, 시신경염, 맨아래구역증후군 등이 특징이다. 수분 통로인 aquaporin-4에 대한 항체가 진단에 특이적으로 알려져 있다. 급성기에는 고용량 스테로이드 정맥 주사, 필요시 혈장교환술까지 시행하고, 재발을 막기 위하여 장기적으로 면역억제제를 투여한다. 이처럼 베르니케뇌병증과 시신경척수염범주질환은 발생 원인, 기전 및 치료 방법이 전혀 다름에도 불구하고 증상 및 영상 소견이 유사한 경우가 있어 진단에 혼란을 초래할 수 있다. 저자들은 베르니케뇌병증으로 오인된 항체 음성 시신경척수염범주질환 증례를 경험하여 이를 보고하고자 한다. | - |
