| dc.description.abstract | 목적: 초기 파킨슨병으로 진단받은 한국인 환자에서 빛간섭단층촬영(optical coherence tomography, OCT) 및 빛간섭단층촬영혈관조영술(OCT angiography, OCTA)을 이용하여 황반부의 신경섬유층 및 신경절세포-속얼기층(nerve fiber layer-ganglion cell inner plexiform layer, NFL-GCIPL)의 두께와 황반부의 미세혈관 변화를 확인하고자 한다. 대상과 방법: 초기 파킨슨병으로 진단받은 22명 43안을 연구에 포함하였으며, 대조군으로 정상인 20명 40안을 설정하였다. 모든 환자는 OCT를 이용하여 황반부의 NFL-GCIPL을 측정하였으며 OCTA를 이용하여 황반의 표층혈관총과 심층혈관총의 혈관밀도를 확인하였다. 결과: NFL-GCIPL 두께는 대조군과 비교하였을 때 대조군은 상측, 하측, 이측, 비측이 각각 94.70 ± 9.35, 93.32 ± 9.16, 90.18 ± 6.32, 93.11 ± 8.75 μm였으며 파킨슨 환자군은 각각 92.05 ± 4.96, 91.32 ± 7.48, 84.74 ± 6.82, 91.32 ± 7.47 μm로 모든 영역에서대조군에 비해 의미 있는 감소 소견을 보였다. 반면 OCTA로 측정한 황반부 표층 및 심층혈관총의 혈관밀도는 두 군의 차이를 보이지않았다. 결론: 본 연구를 통해 한국인 초기 파킨슨병 환자에서 황반부의 미세혈관의 변화보다 신경변성의 손상이 두드러지게 나타남을 알수 있었으며 추후 코호트 연구를 통해 신경변성의 변화를 확인하는 것이 필요할 것이다. | - |
| dc.description.abstract | Рurроsе: To investigate the thickness of the nerve fiber layer-ganglion cell inner plexiform layer (NFL-GCIPL) complex and microvascular macular changes in Korean patients with early Parkinson’s disease using optical coherence tomography (OCT) and OCT angiography (OCTA). Меthоds: Forty-three eyes of 22 patients with early Parkinson’s disease were included. A control group of 20 patients (40 eyes) was also recruited. The thickness of the NFL-GCIPL macular complex was measured using OCT, and the densities of the superficial and deep macular retinal vessels were evaluated via OCTA in all subjects. Rеsults: The NFL-GCIPL thicknesses of the superior, inferior, temporal, and nasal sectors were 94.70 ± 9.35, 93.32 ± 9.16, 90.18 ± 6.32, and 93.11 ± 8.75 μm in the control group and 92.05 ± 4.96, 91.32 ± 7.48, 84.74 ± 6.82, and 91.32 ± 7.47 μm in the Parkinson’s disease group, respectively; all thicknesses were significantly greater in the control group. The superficial and deep retinal vessel densities did not differ between the two groups. Cоnсlusіоns: Neurodegenerative macular changes are more obvious than microvascular changes in patients with early Parkinson’s disease.
