망막동맥대동맥류 파열 후 발생한 내경계막밑 출혈의 특징적 안저와 빛간섭단층촬영 분석

| dc.description.abstract | 목적: 망막동맥대동맥류 파열 후 발생한 황반앞출혈에서 보이는 특징적인 노란색 띠의 빛간섭단층촬영 분석을 통하여 유리체밑출혈과내경계막밑출혈의 구분을 보고하는 바이다. 증례요약: 74세 여자 환자가 3일 전에 갑자기 발생한 좌안 시력저하를 주소로 내원하였다. 좌안의 최대교정시력은 0.1이었으며 좌안 안압은 11 mmHg로 측정되었다. 안저검사에서 좌안의 황반앞출혈 및 망막동맥대동맥류의 파열 부위가 의심되었으며, 황반앞출혈의 주변에 특징적인 노란색 띠가 관찰되었다. 스펙트럼영역 빛간섭단층촬영에서는 돔 모양의 내경계막과 시신경섬유층 사이에 출혈이 관찰되었으며 안저에서 관찰된 노란색 띠는 외경계막의 두꺼운 고반사영역으로 관찰되었다. 내경계막밑출혈 진단 하에 유리체절제술과 내경계막을 벗기고 출혈을 제거하였다. 술 후에 안저에서 보이던 노란색 띠와 빛간섭단층촬영에서 보이는 외경계막의 고반사영역은 사라졌으며, 이후 재발 없이 경과 관찰 중이다. 결론: 황반앞출혈 환자 중 안저에서 보이는 노란색 띠와 빛간섭단층촬영에서 외경계막의 고반사는 유리체밑출혈과 구별되는 내경계막밑출혈의 특징적인 소견으로 진단에 도움이 될 수 있다. | - |
